But why do so few attain what they dream of?

I want to add to that; you won't suddenly become a mean person (a fear a student in my "How to Stop Binge Eating" course expressed) or suddenly become promiscuous (a fear I had when I was obese and single).

You're still going to be the same person, only fitter.

So give yourself permission to succeed. As Ralph Waldo Emerson once said, "Do the thing you fear and the death of fear is certain."
